How the team works
In order to try and provide support to individuals either in their own homes and or communities across Leeds, we have split into two clinical teams - the Blue Team or the Yellow Team. Each young person we work with is allocated to one of these teams. The workers in each team will use a team approach to offer support. This means that a client may receive support from all members of the team at different times. They plan and agree together how best to offer support for each client. It also means that information a client gives you to one member of the team may be shared with other members of the team. Every client will be given the opportunity to discuss confidentiality boundaries. The team will keep client records on everyone we work with. Each client is entitled to have access to their own record.
